Life Styles Supported Living Services
Supported Living Services are funded through Arkansas’ Medicaid Waiver program. To request an application for Medicaid Waiver Services, contact the Developmental Disabilities Services (DDS) Program Manager at the Department of Human Services 870-741-6107. This packet is fairly lengthy. If you need assistance with the paperwork, contact Sources at (479) 442-5600. There are a number of documents that you will need to include with the application, including a current Psychological Evaluation (including an IQ test such as a WAIS-R), an adaptive functioning level report (such as a Vineland) and a current physical from your physician. If your family member has recently attended public school or if he/she is in school, the school should have the documents you need.
Once the packet is received by the Department of Human Services, it will be reviewed to determine if the individual qualifies for services. If the individual does meet the requirements for services, he/she will be put on a waiting list. The time on the waiting list varies but could take several years. Because the state places individuals on a waiting list, it is very important to get your application in as soon as possible. You will be notified by letter when your family member’s name comes up on the waiting list. There will be a choice form included for you to select the agency you want to provide the direct services to your family member, and the agency to provide case management services. You may select the same agency to provide both direct services and case management or select two different agencies. The agency or agencies that you have chosen will schedule a planning meeting PCSP with the individual and family. It will be at this meeting that you will discuss the types of services you want, the schedule that you would like, the goals and objectives that will be addressed and any other consideration prior to the beginning of services.
Once a year, the Medicaid Eligibility Unit of the State will send out a packet of information to verify that the individual is still financially eligible for Medicaid. This form must be returned within 30 days. If you would like, we can assist in that process.

College for Living Services
To participate in Life Styles’ College for Living services, please contact Lindsay Broshears at (479)521-3581 for a tour of the facility and class schedule. College for Living services are billed through Medicaid Adult Development Services. We will schedule a meeting to discuss your interests, goals and objectives and give you the appropriate form for your physician to sign. Classes are offered Monday through Friday from 9:00 until 5:00 and depending on your schedule, location and van availability, we may be able to provide transportation. If you are not approved for Medicaid services, we can also discuss the possibility of private pay options.
For Employment Services
Apply at Arkansas Rehab on Front Street in Fayetteville (479)582-1286. At the first appointment, the counselor will collect a copy of your ID, SS #, any information on income. During the second appointment they will discuss the level of support needed to assist the person in finding a job, and lay out all the options for job assistance. If they determine that Supported Employment (long term support) is needed, and then you may choose a provider and funds will be released to the agency selected.
